To get your card, follow the link below to open a form-fillable PDF.  Complete all the relelvant fields.

Print the document double-sided, flipping on the long edge.  The front and back of the card should line up so that you can cut out the card easily.

Fold the card from the bottom-up, so that the most private information is enclosed inside the other layers and only the Red Cover Page and the Primary Contact Information are visibile.

Laminate the card using a self-sealing business card pouch or even simple packing tape.

Carry a card in your pocket, line-up folder, or ball bag while working.  Keep another copy in your gear bag.

Most importantly, tell your partners that you're carrying this life saving information and encourage them to do so as well!
